Provider Role In The Health And Wellness Program
Providers play an important role in the MO HealthNet Health and Wellness Program. First, providers are able to identify MO HealthNet participants who will benefit from program services. Second, they promote the Program to their patients. Finally, collaboration between providers and the Health and Wellness Program’s care team creates a stronger continuum of care, improved clinical outcomes, and lower costs.
Our goal is to support you in your care of all MO HealthNet participants including healthy and the most fragile individuals – those who are disabled, chronically ill, and elderly – with a coordinated team. The MO HealthNet Health and Wellness Program care team is a crucial part of this network, and in partnership with physicians, physician assistants, nurses, case managers and community organizations, can make a significant impact on your patient’s health and quality of life.
