CareConnection®
CareConnection® is a care management tool used in conjunction with an Internet-based Plan of Care. Health care concerns identified through claims trigger CareConnection® Plans of Care which are accessible to the health care providers. This collaborative electronic health record is designed to facilitate communication between the MO HealthNet Health and Wellness Nurse Health Coach and the participant's health care home. Health care providers can collaborate online via CareConnection® with the Nurse Health Coaches on the needs of the participants. Incentives are available to providers who utilize CareConnection®.
