Participant
The MO HealthNet Health and Wellness Program can help you improve your health. Free and voluntary, the program is for most fee-for-service participants covered by MO HealthNet (Missouri Medicaid).
Most MO HealthNet participants can get support from our dedicated health care team. Through this Program you will receive health and wellness reminders, as well as information about diabetes, asthma, heart disease, sickle cell disease and other conditions.
The Program is not available to participants enrolled in Managed Care.
Enrolled Participants Receive:
- A health care team lead by a registered nurse providing you with support to discuss your health, assess your needs and coordinate care
- Educational materials to encourage the self-management of your health
- A toll-free, 24-hour nurse phone line
- Coordination of community resources such as sources for food, shelter and transportation to reduce barriers to maintaining your health
How Is It Different From The Regular MO HealthNet Program?
The Health and Wellness Program is an additional benefit of MO HealthNet, along with those you already have. Our health care team will help you:
- Understand how to be as healthy as possible
- Know how to prevent problems that can affect your health
- Find a health care home to help you become and stay healthy
Does This Replace My Doctor’s Care?
No. It is very important to keep going to your doctor as usual. The Health and Wellness staff will also work with your doctor to make sure you are receiving the care and resources you need.
How Much Does It Cost?
The Health and Wellness Program is a free benefit to MO HealthNet participants.
